To Enroll

*   Students in levels Pre-Ballet through Level 7 of Ballet, and Jazz 1 through 5, Modern and Hip-Hop (children's) must fill out an enrollment form and a Curriculum/Class Reservation Form which must be agreed upon, signed and approved by parents and instructors.
*   Adult or “open class”*(see schedule) students are required to fill out an enrollment form. No enrollment fee is required.

                                                                   Tuition Payment

*   Tuition for students in the Children/Teen Division is to be paid in 3 installments to be spread equally through the year - September - June. The first payment is due September 1, or upon enrollment for new students. The second and third payments are due on or before December 1 and March 1, even if the student is absent.
*   A $25.00 late fee will be charged on tuition received after the 10th of said months and students may not be allowed to participate until tuition is paid.
*   Adult students may purchase classes at any time to be used within one month of purchase.
*   Payments may be made in person, submitted on the front desk in the tuition box, online banking or by mail to:                                                                                            Professional Ballet School 425 Harbor Blvd. #3 Belmont, CA 94002.                   *   There is a $15.00 penalty for returned checks.

*    PBS accept cash, checks or credit cards.
